PLEASE NOTE
In order to determine the shipping and handling fees
and any applicable sales tax, you'll want to go through the process
of placing items into your shopping cart, then print the shopping
cart page. Then take the information (product stock #'s, prices,
shipping and handling, sales tax, etc.) from the shopping cart
summary page and enter it onto the ORDER SHEET. Alternatively,
you may simply fill out your order information in the shopping
cart, proceed to check out, fill in the appropriate fields, then print
the form to mail your order.
